Personal Information Updates

The form linked below allows students to update or correct certain information in their 海角社区record. Please be sure to review the information below regarding the specific updates and corrections that can be made as well as the documentation requirements. For questions about updating your personal information, you may reach out to One-Stop Student Services.

Please Note: This form and applicable documentation must be submitted in-person to One-Stop Student Services located in . Current/active students, who are unable to visit in person, may email from their 海角社区account a completed and signed form along with legible copies of applicable documentation - including photo ID - to records@unf.edu. Former/inactive students who submit forms and documents from a non-海角社区email are subject to additional identification verification procedures.

  • Legal Name Change

    If you are visiting One-Stop Student Services in person or emailing from a non-海角社区email address to update your legal name, a completed Personal Information Update Form, documentation, and photo ID must be provided.

    If you are emailing from your 海角社区email address, photo ID verification is not required. Only the completed Personal Information Update Form and documentation are required to make the update.

    Examples of documentation showing your updated legal name include:

    • Valid Driver's License
    • Valid State ID
    • Social Security card
    • Valid Passport
    • Court orders
    • Divorce papers

     

    Please Note: If you are a current 海角社区employee submitting a name change request (A&P, USPS, OPS, graduate assistant, or student employee), you must also complete a Human Resources Name Change and a W-4 form. Submit these forms to the Office of Human Resources (HR) along with an original Social Security card reflecting your new, legal name. To obtain the HR Name Change form, visit the 海角社区Human Resources webpage or stop by Human Resources, .

  • Preferred First Name Change

    If you are visiting One-Stop Student Services in-person or emailing from a non-海角社区email address to update your preferred first name, both a completed Personal Information Update Form and photo ID must be provided to verify your identity.

    If you are emailing from your 海角社区email address, photo ID verification is not required. Only the completed Personal Information Update Form is required to make the update.

    Please Note: Your preferred first name will only appear on 海角社区email, Canvas, and the 海角社区Directory. Please refer to our Preferred Name and Pronouns webpage for more information on UNF's "Preferred Name and Pronoun Use" policy.

  • Social Security Number (SSN) Correction

    To verify the associated Social Security Number (SSN) that 海角社区has on file, please follow the below steps:

    • Log in to
    • Select Student under the left "My Resources" menu
      • If you are on a mobile device, select the Student tile
    • Select the Banner Self Service button
    • Select Student Records
    • Select Social Security Number Validation
      • Enter your Social Security Number for verification

    If the Social Security Number on file is incorrect, please see the below instructions.

    If you are visiting One-Stop Student Services in person or emailing from a non-海角社区email address to update your social security number, a completed Personal Information Update Form, your Social Security card, and photo ID must be provided.

    If you are emailing from your 海角社区email address, photo ID verification is not required. Only the completed Personal Information Update Form and your Social Security Card are required to make the update.

    Please Note: Social Security cards must be signed to be accepted.

  • Date of Birth Correction

    If you are visiting One-Stop Student Services in person or emailing from a non-海角社区email address to update your date of birth, a completed Personal Information Update Form, documentation, and photo ID must be provided.

    If you are emailing from your 海角社区email address, photo ID verification is not required. Only the completed Personal Information Update Form and documentation are required to make the update.

    Examples of documentation showing your correct date of birth include:

    • Valid Driver's License
    • Valid State ID
    • Valid Passport
    • Birth certificate
    • etc.
  • Gender Update

    If you are visiting One-Stop Student Services in person or emailing from a non-海角社区email address to update your legal gender, a completed Personal Information Update Form, documentation, and photo ID must be provided.

    If you are emailing from your 海角社区email address, photo ID verification is not required. Only the completed Personal Information Update Form and documentation are required to make the update.

    Examples of documentation showing your legal gender include:

    • Valid Driver's License
    • Valid State ID
    • Valid Passport
    • Court orders
    • etc.
  • Personal Pronoun Update

    If you are visiting One-Stop Student Services in person or emailing from a non-海角社区email address to update your personal pronoun, both a completed Personal Information Update Form and photo ID must be provided to verify your identity.

    If you are emailing from your 海角社区email address, photo ID verification is not required. Only the completed Personal Information Update Form is required to make the update.

  • Address Update

    To view and update your address, please follow these steps:

    • Log in to
    • Select Student under the left "My Resources" menu
      • If you are on a mobile device, select the Student tile
    • Select the Banner Self Service button
    • Select the Personal Information tab
    • Select Update Address(es) and Phone(s)

    If you do not have access to your myWings account, please follow the below instructions.

    If you are visiting One-Stop Student Services in person or emailing from a non-海角社区email address to update your street address, both a completed Personal Information Update Form and photo ID must be provided to verify your identity.

    If you are emailing from your 海角社区email address, photo ID verification is not required. Only the completed Personal Information Update Form is required to make the update.

  • Telephone Number Update

    To view and update your telephone number, please follow these steps:

    • Log in to
    • Select Student under the left "My Resources" menu
      • If you are on a mobile device, select the Student tile
    • Select the Banner Self Service button
    • Select the Personal Information tab
    • Select Update Address(es) and Phone(s)

    If you do not have access to your myWings account, please follow the below instructions.

    If you are visiting One-Stop Student Services in person or emailing from a non-海角社区email address to update your telephone number, both a completed Personal Information Update Form and photo ID must be provided to verify your identity.

    If you are emailing from your 海角社区email address, photo ID verification is not required. Only the completed Personal Information Update Form is required to make the update.

  • Personal Email Address Update

    If you are visiting One-Stop Student Services in person or emailing from a non-海角社区email address to update your personal email address, both a completed Personal Information Update Form and photo ID must be provided to verify your identity.

    If you are emailing from your 海角社区email address, photo ID verification is not required. Only the completed Personal Information Update Form is required to make the update.

  • Emergency Contact Update

    If you are visiting One-Stop Student Services in person or emailing from a non-海角社区email address to update your emergency contact, both a completed Personal Information Update Form and photo ID must be provided to verify your identity.

    If you are emailing from your 海角社区email address, photo ID verification is not required. Only the completed Personal Information Update Form is required to make the update.
